Prescription Refill Request
If you need a prescription refill, you can submit that request directly by clicking the 'Refill Request' button on the top right corner of the Adaptive Minds website. This online form will gather pertinent information regarding your prescription request and send it to your provider for processing. Please allow up to 3 business days for refill requests to be sent to your pharmacy.
Starting June 1st, we will be using this process only. We will only be able to guarantee timely fulfillment of prescription requests if they are submitted via the refill request form. Thank you for your collaboration as we transfer to this process to ensure no refill requests are missed.
About Appointment Change Request
In simplifying our appointment rescheduling process, we have started to move all appointment cancellation requests to a simple Appointment Change form on our website. The Adaptive Minds team will get back to you to confirm the change in your appointment.
Please be aware that a 24-hour notice is required to change or cancel an appointment. Late cancellations will be subject to a fee.
